Important InfoTERMS /CONDITIONS: Assessment fees are payable at time of initial appointment. Therapy fees are payable within 7 days of invoicing. Fees will apply to cancellations given with less than 24 hours notice. A late fee applies to bills left unpaid more than 14 days. Any collection costs incurred will be charged to the client. CONSUMER RIGHTS: Client/parents/caregiver /Whanau are advised of their rights referring to Human Rights Act 1993, Health and Disability Act 1994. Therapy for Children promote best practice and abide by all relevant legislation e.g. Privacy Act 1993, Health and Safety in Employment Act 1992 and Amendments 2003. (Detailed information is displayed on wall posters, brochures and in the procedural manual at Therapy for Children). Following discussion of your rights you will be asked to sign an “Informed Consent” statement before any assessment or treatment is undertaken. CULTURAL CONSIDERATIONS: Therapy for Children is committed to continuously improving its cultural competency at both an organisational and individual level. We recognise and respond to all cultural considerations. The needs of the client, their families/whanau, caregivers/relevant others, who are involved in the assessment process are considered paramount. We will listen to and understand the person/family in their context. Their rights will be preserved; confidentiality honoured and needs respected at all times. PRIVACY: Therapy for Children will maintain the privacy of all clients in accordance with the Privacy Act 1993. Information regarding clients will only be requested and recorded if it is relevant to their therapeutic needs. All information recorded or requested for the purpose of needs assessment or therapeutic intervention will be undertaken with the consent of the client and/or their parent/caregiver. Copies of notes and other recorded information will be made available to the client/caregiver at their request providing the information is exempt under The Privacy Act 1993. ACCIDENTS/EMERGENCY: In the event of serious injury and to ensure that appropriate medical intervention is sought all staff know the location of the First Aid Box on the premises. All incidents are recorded in our Accident Register. In the event of a serious accident or injury/medical emergency, medical assistance/intervention will be sought immediately by dialling 111. In case of fire, evacuation procedures are clearly displayed on premises. PROFESSIONAL PRACTICE: Therapy for Children Employees/Contractors hold current practicing certificates in accordance with the requirements of their professional organisation. Current practising certificates are sighted by the Therapy for Children Company Director annually. Therapy for Children Employees/Contractors are accountable for maintaining excellent standards of professional practice. |
